Fault tree analysis is a deductive method to assess the probability of incidents and reduce risk of failure.  Reducing the probability of an incident generally reduces the risk of system failure.  The risk of workplace injuries decreases (or increases) as potential root causes (risks) are controlled or eliminated. The Adaptive Safety Gemba is simple yet it has proven to be an extremely effective approach to reducing workplace injuries and illnesses.  Take the Adaptive Safety Gemba – walk the shop floor or worksite – and identify as many hazardous conditions and hazardous acts as possible.  Find it – Fix it – Repeat.  Our expertise is not in the fault tree – it is the “what” and “how” of the Adaptive Safety Gemba.
